According to the new WHO classification of 2019, we reclassified a large single-center cohort of 1004 CRC according to WHO grading, tumor budding activity and histological subtype. We investigated the interaction of these parameters, their relationship to tumor stage and their significance for patient survival. Our analysis of the key morphological parameters focused on CRC subtyping as a strong stage-independent prognostic parameter and identified tumor budding in particular as the strongest histomorphological prognostic factor, which is superior to the conventional WHO grading system.
