Einleitung: Die dentale Implantologie ist seit vielen Jahren ein fester Bestandteil der Wiederherstellung des stomatognathesn Systems. Feingewebliche Erkenntnisse über diese Verfahren kommen überwiegend aus tierexperimentellen Studien. Histologische Langzeituntersuchungen am Menschen sind sehr selten. In der vorliegenden Arbeit werden funktionell eingeheilte Implantate am Menschen untersucht. Material und Methode: Es wurden an 3 Humanpräparaten insgesamt 9 Implantate untersucht. Die funktionelle Verweildauer lag zwischen 5 und 13 Jahren. Alle Implantate waren im Unterkiefer inseriert. Alle Proben wurden im µCT gemessen und histologisch aufbereitet. Die Proben wurden histomorphometrisch ausgewertet. Der periimplantäre Knochen wurde dazu in 9 Messzonen, 3 je Hals, Körper und Apex, eingeteilt. Es wurden der Knochen-Implantat-Kontakt (BIC), das relative Knochenvolumen und anhand von 100 x 100µm großen Messfeldern die Osteozytendichte bestimmt. Ergenbis: Alle Implantate zeigten sich klinisch stabil eingeheilt. Der BIC lag zwischen 17,09% und 75,91%. In den 9 Messzonen lag das relative Knochenvolumen zwischen 0,49% und 84,05% und die Osteozytendichte zwischen 0 und 10 pro Messfeld. Zusammenfassung: Langzeituntersuchungen dentaler Implantate am Menschen zeigen, dass einerseits ein Implantat die biologische Qualität des periimplantären Knochens unabhängig vom BIC beeinflusst und andererseits die Knochentrabekel eine funktionelle Ausrichtung zur Implantatoberfläche zeigen.      «

Introduction: Since many years dental implants are a major part in the rehabilitation of the stomatognathic system. Histological findings are made mostly in animal trials. Long-term histological results on humans are very rare. The presented work shows results of functionally integrated dental implants on humans. Material and Method: 3 human cadavers with a total number of 9 implants were evaluated. The functional integration period was between 5 and 13 years. All implants were placed in the mandible. The specimens were measured by µCT and prepared histologically. A histomorphometrical evaluation was performed. The periimplant bone was divided in 9 areas of intrest (aoi), 3 per cervix, body and apex. The bone-implant-contact (BIC), the relative bone volume and the count of osteocytes per region of intrest (roi) 100 x 100 µm was measured. Results: All implants were clinically stabile. The BIC was between 17,09% and 75,91%. The relative bone volume in the 9 aoi was between 0,49% und 84,05% and the count of osteocytes between 0 und 10 per roi. Conclusion: Longterm results of functionally loaded dental implants on humans show that the implant influences the biological quality of the periimplant bone independently from the BIC and that the bone structure shows a functional orientation to the surface of the implant.      «
