Verletzungen des hinteren Kreuzbandes (HKB) sind verhältnismäßig seltene Kniebinnenverletzungen. Bei bleibender Gelenkinstabilität nach Ruptur des HKB kann ein operativer Ersatz notwendig werden. Studien zeigen, dass der Ersatz der beiden funktionellen Hauptbündel der Kreuzbandfasern (Double-Bundle-Technik)Vorteile gegenüber einem Transplantat aus nur einem Faserbündel hat. Für einen Kreuzbandersatz gilt die physiologische Transplantatführung ohne exzessive Längen- und Spannungsveränderung als wichtig. In der vorliegenden Studie wurde für den Ersatz des HKB in Double-Bundle-Technik innerhalb der Ansatzstellen an Femur und Tibia nach Fixationsstellen für die zwei Transplantat-Bündel gesucht, deren Distanzänderung während Kniebeugung am geringsten ist. Nach Durchführung von Fadenmessversuchen an 7 Leichenknien kann für beide Hauptbündel eine Fixationsstelle am postero-superioren Rand der Insertionsfläche am Femur empfohlen werden.     «

Isolated injuries of the posterior cruciate ligament (PCL) are relatively rare injuries of the knee joint. In case of persisting instability of the knee joint surgical reconstruction of the PCL can become necessary. Studies show advantages for the reconstruction of both main fibre-bundles of the PCL (Double-Bundle-Technique) in comparison to a transplant consisting of only one bundle. For successful reconstruction of the cruciate ligaments a physiological placement of the transplant without excessive change in length is important. In this study insertion points for a Double-Bundle-Reconstruction of the PCL within the natural insertion areas on femur and tibia were examined. For each of the two bundles insertion points were determined that show the least change in distance during flexion of the knee joint. Testing was executed on 7 cadaver knees. After statistical evaluation of the data for both bundles a fixation near the posterior-superior margin of the native insertion on the femur can be recommended.      «
