Hintergrund: Stellenwert der adjuvanten Radiotherapie (RT) und der Radiochemotherapie (RCHT) im Therapiekonzept des operabelen Rektumkarzinoms. Patienten und Methode: Retrospektive Datenverarbeitung von 165 Patienten vom 01.01.1984 bis 31.05.1997. Ergebnisse: Die tumorspezifische 5-Jahresüberlebensrate liegt bei 69,0%. Es Konnte, wahrscheinlich wegen der geringen Patientenzahl, kein signifikanter Unterschied zw. RT und RCHT im Bezug auf die 5-Jahres-Überlebensrate und Lokalrezidivrate festgestellt werden. Schlussfolgerung: Im Stadium II und III kann die adjuvante RCHT noch als Stand angesehen werden. Die Lokalrezidivquote kann durch systematischen Einsatz der totalen mesorektalen Exzision und die Überlebensrate durch Anwendung modernerer und effektiverer Chemotherapieschemata sicher noch deutlich optimiert werden. Die präoperative RCHT wird als neuer Standard empfohlen.     «

Background: Significance of adjuvant radiation therapy (RT) and combined radio-chemotherapy (RCHT) within the therapeutical concept of operable rectal carcinoma. Patients and Methods: Retrospective analysis of 165 cases from Jan. 1st 1984 to May 31st 1997. Results: The tumour-specific five-year survival rate was 69.0%. No significant difference in five-year survival rate and the rate of local recurrence was found between the RT- and the RCHT-group, probably because of the small number of patients in each group. Conclusion: Adjuvant RCHT can still be considered the standard of treatment in stages II and III. The local recurrence rate could be improved through consistent use of total mesorectal excision while the survival rate might improve with modern chemotherapeutic agents. Pre-operative RCHT is recommended as the new standard.     «
