Inflammatory bowel disease (IBD) frequently result in an impairment of the quality of life. A reduction of life-expectancy has been discussed. However, surveys of intensive care patients with IBD are lacking. Therefore, all patients with IBD among 6.071 consecutive admissions to the intensive care unit (ICU) of Klinikum Rechts der Isar, Munich, were analysed. Four main reasons caused admission to ICU: Complications of IBD, sepsis, thrombosis or embolism and extraintestinal manifestations. During ICU stay or within 28 days afterwards, 14% of the patients with IBD died due to complications or treatment of IBD. All non-survivors died of sepsis and they were significantly older than survivors. Further risk factors for organ failure and mortality were identified. High potential immunsuppressive drugs should be applied strictly according to the consensus-guidelines of DGVS.
