In the acute myocardial infarction there is an increased concentration of inflammatory mediators in the myocard and in the serum of patients with myocardial infarction. For a better understanding of this mechanism we did a gene expression analysis of RNA from circulating leukocytes of patients with myocardial infarction during the first days at hospital. Proinflammatory changes with immunoassay were confirmed for IL-1, IL-6, IL-8, IL-10, IL-12, VEGF and RANTES. The results of this analysis showed a persistent inflammatory response over 5-10 days in connexion with myocardial necrosis, with increased cardiac enzymes and with the acute phase proteins. In addition we did a gene expression analysis of RNA from endothelial cells after in vitro plasmaincubation of patients with myocardial infarction. According to these results theres is an persistent activation of leukocytes and proinflammatory processes in the acute myocardial infarction. Further investigations of the underlying mechanisms could show new therapeutic possibilities.
