Magnetic resonance imaging (MRI) is valued highly for the diagnosis and progress monitoring of multiple sclerosis (MS). The current scientific discussion raises the question of how the so called clinical/MRI paradox arises, and what, on the basis of the MRI, are the implications for diagnosis, prognosis and treatment progress. An evaluation of the literature confirms the following: The MRI enables the early diagnosis of MS. The MRI is valuable for prognosis only in early stage of MS. The benefit of a routine follow up of MRI to monitor progress must be put into question.
