Die repetitive transkranielle Magnetstimulation (rTMS) wird erfolgreich in der Erforschung der Pathophysiologie neurologischer und psychiatrischer Erkrankungen eingesetzt. Dabei wird in einer Stimulationsspule ein rasch wechselndes magnetisches Feld erzeugt, das im angrenzenden Hirnareal zu einer Erregung kortikaler Neurone führt. In zwei Versuchsreihen wurden die Effekte der rTMS auf die motorische Funktion und die kortikospinale Exzitabilität bei Patienten mit Morbus Parkinson untersucht. Die Stimulation bestand aus je 15 Serien einer unterschwelligen 5-Hz rTMS. In Versuchsreihe A wurde bei 12 Patienten mit Morbus Parkinson die motorische Symptomatik vor und nach einer unterschwelligen 5-Hz rTMS des primären sensomotorischen Handareals (Verum) und des mittleren frontalen Kortex (Kontrollbedingung) beurteilt. Die motorische Symptomatik wurde zum einen mit einer standardisierten klinischen Untersuchung erfasst, zum anderen anhand von ballistischen Zeigebewegungen, bei denen Reaktionszeit, Bewegungszeit und kinematische Parameter bestimmt wurden. In Versuchsreihe B wurden bei 12 Patienten und bei zehn gesunden Kontrollen die magnetisch evozierten Potentiale in Muskelruhe bei Reizintensitäten von 100 -160% sowie unter isometrischer Vorinervation bei einer Reizintensität von 130% jeweils vor und nach der unterschwelligen 5-Hz rTMS über dem primären sensomotorischen Handareal abgeleitet. Dabei wurde die motorische Schwelle, kortikale Latenz, Amplitude und postexzitatorische Innervationsstille bestimmt. In Versuchsreihe A zeigte nur die Verumbedingung einen signifikanten, die Stimulation überdauernden Effekt auf die Handmotorik. Alle drei Kardinalsymptome Bradykinese, Rigor und Tremor besserten sich insbesondere an der kontralateralen oberen Extremität. Zudem nahm die Bewegungszeit signifikant ab und der Automatisierungsgrad ballistischer Bewegungen zu. In Versuchsreihe B gab es sowohl gruppenspezifische als auch gruppenunabhängige Effekte. So zeigten die Patienten eine selektive Verlängerung der postexzitatorischen Innervationsstille als Ausdruck einer erhöhten intrakortikalen inhibitorischen Aktivität. Die gesunden Kontrollen zeigten eine Abflachung der Input-Output-Kurve im Sinne einer reduzierten Verstärkerfunktion im kortikospinalen System. In beiden Gruppen nahm der MEP-Amplitudenquotient Vorinnervation zu Muskelruhe zu. Somit ermöglicht die rTMS, durch Vorinnervation des Zielmuskels die kortikospinale Erregbarkeit zu steigern. Diese Untersuchungen belegen einen kurzzeitigen günstigen Effekt einer unterschwelligen 5-Hz rTMS des primären sensomotorischen Handareals auf die Handfunktion bei Patienten mit Morbus Parkinson. Ob dieser konditionierende Effekt eine Therapieoption darstellt muss derzeit noch offen bleiben.     «

Repetitive transcranial magnetic stimulation (rTMS) is an effective method for studying the pathophysiology of neurological and psychiatric diseases. A rapidly changing magnetic field in the coil induces an excitation of cortical neurons in the area of the brain closest to the coil. The effects of rTMS on motor function and corticospinal excitability in patients with Parkinson´s disease were examined in two trials (A and B). In both trials the subjects underwent 15 series of a sub-threshold 5-HZ rTMS. In experiment A the motor symptoms before and after a sub-threshold 5-Hz rTMS of the motor handarea (verum) and of the middle frontal cortex (control) were recorded. The motor symptoms of the upper extremity were investigated by a clinical standard examination. Furthermore, the subjects performed ballistic aiming movements, which were analysed for reaction time, motion time and kinematic parameters. In experiment B before and after a sub-threshold 5-Hz rTMS of the motor hand area the motor evoked potentials (MEP) of 12 patients with Parkinson´s disease and ten healthy controls were recorded without any muscle activity of the first interosseus digiti muscle at intensities of the magnetic stimulator from 100% to 160%, and again with a slight muscle activity (10% percent of maximal activity) at an intensity of 130%. The motor threshold, cortical latency, amplitude and silent period of the MEP´s were analysed. In trial A significant short term lasting effects on motor symptoms of upper extremities were observed only after a sub-threshold 5-Hz rTMS of the motor hand area. The main symptoms bradykinesia, rigidity and tremor improved especially at the contralateral upper extremity. A significant decrease in motion time was noticed and the ballistic aiming movements showed a significantly higher degree of automatisation. In trial B group-dependent as well as group-independent effects were observed. Only the patients showed an increase in the silent period according to an enlargement of intracortical inhibitory activity, whereas healthy controls revealed a lower input-output curve. This suggests that the amplifying function in the corticospinal system is reduced. In both groups the quotient of muscle-innervation by the muscle-silence of the amplitudes increased. Thus rTMS makes it possible to increase the cortico-spinal excitability by muscle innervation. These results confirm that there is a lasting short-term benefit of a sub-threshold 5-Hz rTMS of the motor cortex of the hand on manual function in patients with Parkinson´s disease. Whether this conditioning effect has a therapeutical importance can not be answered yet.     «
