During adjuvant breast cancer radiation therapy, a 5 mm PTV margin is frequently employed around the internal mammary artery. This study assesses the safety and efficacy of this standard 5 mm safety margin compared to a modified margin that excludes lung tissue, using retrospective dose simulations in 20 patients. The findings show that while the standard margin ensures reliable dose coverage, the application of the modified margin leads to significant dose drops in the target area, indicating that the implementation of a reduced margin is not recommended.
