The effects of different insulin (I) therapy schemes on myocardial and vascular function were prospectively studied both fasting and postprandial (pp) in 128 I dependent patients with type 2 diabetes, 61 of which had had homogenous medical control but the remainders not. Within the former individuals, only therapy with mixed I was associated with significantly increased pp glucose and lower diastolic cardiac function in spite of similar metabolic and cardiac data compared to multiple daily injection or supplemental rapid I, pointing to the importance of pp glucose control.
