Abstract:
Der Stellwenwert der autologen osteochondralen Transplantation (OATS) an der Patella ist wegen fehlender Behandlungsdaten noch unbekannt. Diese klinische Studie stellt die bisher größte Patientengruppe nach Patella-OATS vor. Von 4/97 bis 3/02 wurden 29 Patienten im Alter von 33,5 J. [16-59] wegen patellärer Knorpeldefekte mit OATS behandelt. 24 Patienten wurden klinisch und kernspintomographisch durchschnittlich nach 30,7 Mo [4-64] untersucht. Der Lysholm-Score stieg von 55,1 auf 81,2 und der Tegner-Score von 2,9 auf 4,5 signifikant an (p<0,001). Der Schmerz reduzierte sich von 6,1 auf 3,0 (VAS) signifikant (p<0,001). Im MRT wurden die OATS-Zylinder in 83,3% weitgehend kongruent zur Oberfläche und ohne wesentlichen Knorpelverlust integriert. Die Patella-OATS ist eine komplexe Therapie, die für umschriebene Knorpeldefekte gute mittelfristige Ergebnisse zeigt. Zur Etablierung dieser Technik müssen dennoch Langzeitergebnisse abgewartet werden.

The significance of autologous osteochondral transplantation (OATS) at the patella is due to lacking treatment results unknown. This clinical study presents the outcome of a, up to date largest patient collective following Patella-OATS. Between 4/97 and 3/02 29 patients with a mean age of 33.5 y. [16-59 y.] were treated with OATS at the patella. A follow-up evaluation with a detailed clinical and MRI examination after 30,7m [4-64] was available in 24 patients. The Lysholm Scores improved from 55,1 to 81,2 and the Tegner Score from 2.9 to 4.5 significantly (p<0.001). The pain score (VAS) reduced from 6.1 to 3.0 significantly (p<0.001).The MRI showed in 83,3% cases the Cylinders largely congruent to the surface and without any massive defects to the cartilage integrated. Patella-OATS is a demanding operative option for the treatment of patellar cartilage defects and offers good mid-term success rates. However, to establish this method we have to wait for long-term results.
