C-terminal agrin fragment (CAF), cleavage product of agrin, was previously correlated with kidney function in renal transplant patients. In this work we found a statistically significant association of early post transplantation serum CAF levels of 105 renal transplant recipients with all-cause mortality and doubling of proteinuria. Concerning the prognostic value for graft failure and combined endpoints CAF outperformed eGFR being more precise. Taken together CAF appears to be a new promising biomarker for the early postoperative risk assessment of long-term-outcome and the unfavorable development of proteinuria in renal transplant patients.
