Abstract:
Ziel dieser Doktorarbeit war die Erstellung und Validierung eines Scores zur Vorhersage der gesundheitsbezogenen Lebensqualität nach elektiven Operationen der HWS anhand präoperativer psychopathologischer Kennwerte. Im Rahmen der statistischen Auswertung konnten das Alter (p=0,010), der SF-36 Mental Composite Score (p=0,024) sowie der Anxiety Sensitivity Score (p=0,012) als signifikante Prädiktoren für eine schlechtere Lebensqualität identifiziert werden. Diese Variablen wurden zur Erstellung des Scores verwendet. Es wurden drei prognostische Gruppen erstellt. Nach Anwendung des Scores in der Bildungs- als auch Validierungsgruppe konnte in allen drei prognostischen Gruppen eine zufriedenstellende Übereinstimmung erreicht werden. Der Score scheint somit valide und reproduzierbar.

The aim of this dissertation was to define parameters by retrospective data analysis that could predict the change in quality of life one year postoperatively. A multivariate regression analysis of the Score Building Group identified age (p=0,010), SF-36 Mental Composite Score (p=0,024) and Anxiety Sensitivity Scores (p=0,012) as significant predictors of a decrease in quality of life one year postoperatively. These factors were included in the score. Three prognostic groups were designed. When comparing the assignments to the three prognostic groups, it becomes obvious that the created score provides similar results in the building as well as in the validation group. Thus, the risk score seems valid and reproducible.
