Abstract:
Bei 100 Oropharynxkarzinompatienten wurde das posttherapeutische funktionelle Outcome untersucht. Die Hälfte der Patienten wurde operativ via Mandible Split und Defektdeckung mittels Radialistransplantat sowie adj. Radio(chemo)therapie versorgt. Die andere Hälfte erhielt eine primäre Radio(chemo)therapie. Im Wasserschlucktest, dem primären Endpunkt dieser Studie, zeigte sich bei den primär konservativ Therapierten eine bessere Performance als in der chirurgisch versorgten Gruppe. Mit der modifizierten Pharynxrekonstruktionsmethode war die Schluckzeit jedoch mit der der konservativen Gruppe vergleichbarer. Daneben wurden nasale Penetration, Xerostomie sowie Artikulation und Lebensqualität analysiert.

We assessed the functional outcome of 100 patients with oropharyngeal squamous cell carcinoma. 50 patients received a transmandibular resection with a radial forearm flap reconstruction and an additional chemoradiotherapy (TMR+CRT) compared to 50 patients receiving primary chemoradiotherapy (pCRT). In the water swallowing test, the primary endpoint of the study, the patients treated with pCRT showed a better performance. However, after modifying the surgical reconstruction technique the swallowing performance was more comparable to the pCRT group. Furthermore we assessed nasal penetration, xerostomia, speech and quality of life.
