Die prospektive Studie untersuchte an 107 Intensivpatienten den Frühmarker des akuten Nierenversagens „Neutrophilengelatinase-assoziiertes Lipocalin“ (NGAL) im Urin bezüglich seiner Assoziation mit dem Outcome sowie dem Vorlast-Status. Die Höhe von NGAL unterschied sich signifikant zwischen Patienten mit normwertiger, erhöhter und erniedrigter Vorlast, gemessen als globaler enddiastolischer Volumen-Index (GEDVI). Hohe Werte von NGAL waren signifikant assoziiert mit Mortalität und dem Bedarf an Nierenersatzverfahren. Direkt bei Aufnahme auf die Intensivstation war NGAL diesbezüglich den etablierten Nierenmarkern Kreatinin und Harnstoff im Serum prognostisch überlegen.

The purpose of this prospective study was to examine associations between „neutrophile gelatinase associated lipocalin“ (NGAL), a biomarker for the early prediction of acute kidney injury and the outcome as well as the preload in 107 critically ill patients. NGAL levels were significantly different in patients with normal, increased and decreased preload, measured as global enddiastolic volume index (GEDVI). High levels of NGAL were associated with mortality and the requirement of renal replacement therapy. The prospective value of NGAL exceeded serum creatinine and serum blood urea nitrogen at the time of admission to the intensive care unit.
