Diese Arbeit beschreibt den ersten kliniktauglichen Entwurf und die Realisierung eines Verfahrens und eines Medizingeräts für die Leistungssteuerung von chirurgischen Instrumenten für die Hals-, Nasen- und Ohrenchirurgie und für die dentale Implantologie. Das Prinzip der Leistungssteuerung (Navigated Control) wurde erstmalig klinisch eingesetzt und evaluiert. Vor dem Eingriff erstellt der Chirurg eine Planung anhand von CT-Daten. Wenn das chirurgische Instrument sich während der Operation an der falschen Position im Patienten befindet, wird die Leistung des Instruments durch das Medizingerät verringert. Dafür misst das Medizingerät die Position des Instruments und des Patienten während der Operation. Die Leistung eines Bohrers oder eines Shavers wird positionsabhängig und phasenabhängig gesteuert. Sensible Strukturen wie Nerven können auf diese Weise beim Bohren von Implantatbetten und beim Abtrag von Polypen besser geschützt werden, und das Operationsergebnis kann verbessert werden.

This thesis describes the first clinically suitable conception and realization of a method and of a medical device for the power control of surgical instruments for Ear, Nose and Throat Surgery and for Dental Implantology. For the first time, the principle of position dependent power control (Navigated Control) is clinically applied and evaluated. Before the surgery, the surgeon creates a surgical planning based on CT data. If the surgical instrument is positioned wrong inside of the patient during the surgery, then the power of this surgical instrument is reduced by the medical device. Therefore, the medical device measures the position of the instrument and of the patient during the surgery. The power of a drill or of a shaver is then controlled depending on its position and on the state of the surgical intervention. Thus, sensitive structures are actively protected during the drilling of implant beds in dental implantology and during the removal of polyps, and the surgical outcome can be improved.
