Aufgrund der engen Nachbarschaft vieler lebenswichtiger Strukturen in einem kleinen Operationsgebiet ist ein chirurgischer Eingriff im Mittelohr- und Felsenbeinbereich anspruchsvoll. Dabei ist unter anderem der schützenswerte Gesichtsnerv (Fazialis) einer hohen Verletzungsgefahr ausgesetzt. In dieser Arbeit wird der neuartige Ansatz zur Lokalisierung des Fazialis und Leistungssteuerung einer chirurgischen Fräse mithilfe von Nervmonitor-Daten (Elektromyographie, EMG) für HNO-Chirurgie weiterentwickelt und ein darauf basierendes chirurgisches Assistenzsystem entwickelt. Die intraoperative Nervlokalisierung wird durch eine Datenfusion von EMG-Daten bei elektrischer Stimulation und von Positionsdaten eines getrackten Nervenstimulators realisiert. Bei der Leistungssteuerung der stimulierenden Fräse werden vor allem die ermittelten Positionsdaten des Fazialis und der Fräse, die EMG-Daten und die Stimulationsstromstärke berücksichtigt.

Due to the close proximity of many vital structures in the small operating field, surgery in the middle ear and petrous bone is challenging. Among other things, the facial nerve is exposed to a high risk of injury. In this thesis the novel approach to localize the facial nerve and to control the power of a surgical drill using EMG data from the nerve monitor for ENT surgery was further developed and a surgical assistance system was designed based on it. The intraoperative nerve localization is implemented through a data fusion of evoked EMG data with electrical stimulation and position information of a tracked nerve stimulator. The power control of the tracked stimulating surgical drill takes into account different data, especially the determined position data of the facial nerve and the drill, the EMG data and the intensity of the stimulation current.
