In einer prospektiven Studie wurden insgesamt 174 Patienten mit einer Fraktur des proximalen Humerus mit komplikationslosen Verläufen erfasst. Es wurden 20 Patienten nach Schultergelenksersatz, 36 Patienten nach Minimalosteosynthese, 106 Patienten nach T-Plattenosteosynthese und 12 Patienten nach Implantation eines proximalen Humerusnagels untersucht. Alle Patienten mit nachgewiesenen Komplikationen wurden aus der Studie ausgeschlossen. Nach Frakturversorgung kam es bei allen Patienten zu einer postoperativen Akut-Phase-Reaktion, die anhand des Parameters C-reaktives Protein gemessen wurde. Die CRP-Bestimmungen erfolgten präoperativ und am 1., 2., 4. und 7. Tag postoperativ. Maximale CRP-Werte wurden nach jeder Operation am 2. postoperativen Tag beobachtet, danach folgte ein rascher Abfall der Werte am 4. und 7. Tag. Je nach Frakturversorgungstyp wurden unterschiedlich hohe CRP-Werte gemessen, wobei die Höhe der Peaks mit dem Umfang der Eingriffe korrelierte. Je höher das CRP-Level, desto größer ist das Operationstrauma. Bei der Prothesenversorgung wurden signifikant höhere Werte beobachtet als bei Minimal-,T-Plattenosteosynthesen und Implantationen von proximalen Humerusnägeln. Die signifikant niedrigsten Werte zeigten sich bei den Minimalosteosynthesen. Die hier ermittelten CRP-Werte beweisen eindeutig das Operationstrauma, wobei der CRP-Peak am 2. postoperativen Tag am besten geeignet ist zur Abschätzung der traumatischen Belastung für den Patienten. Bei der Auswahl der geeigneten Operationsmethode sollte deswegen die Schwere des Traumas und die damit verbundene Belastung für den Patienten miteinbezogen werden.     «

174 patients with proximal humeral fractures and uncomplicated courses were prospectively studied. 20 patients after shoulder arthroplasty, 36 patients after minimal osteosynthesis, 106 patients after T-Plate osteosynthesis and 12 patients after intramedullary humeral nailing were recorded. All patients with complications were excluded out of the study. After operative fracture treatment all patients showed a postoperative acute phase response which was characterized by determination of C-reactive protein levels. Serum CRP levels were determined pre-operatively, on the 1st, 2nd, 4th and 7th day after operation. There was a rise of CRP levels with a maximum on the second day after operation, rapidely decreasing on the 4th and 7th day. Depending on the type of fracture treatment we recorded different CRP levels. We found a correlation between CRP peak and operative treatment. The CRP response was related to the surgical trauma. Significantly higher CRP levels were observed after shoulder arthroplasty than after minimal osteosynthesis, T-Plate osteosynthesis and intramedullary humeral nailing. The lowest CRP levels were significantly observed after minimal osteosynthesis. The CRP response measured in this study was clearly affected by the severity of the trauma. CRP peak on the 2nd postoperative day should be used to assess the severity of traumatic injury for the patient.     «
