Eight parients suffering from hairy cell leukemia were treated with 2-chlorodeoxyamin (2-CDA) at a dosis of 0,09 mg/kg body weight from September 1993 to November 1997. Their first main symptoms were fatigue (7/8), night sweat (5/8) and bleeding tendency (4/8). Four of the eight patients were pretreated with interferon alpha. Splenectomy was performed with one patient as first line therapy, with another as emergency treatment after rupture of the spleen. After a survey of 40 months (range 22 to 56 months) four patients showed a complete remission. Two patients had a clinical and hematological remission but still a minimal residual disease by bone marrow histology and two patients first had a good partial remission but after a survey of 29 and 33 months they relapsed. Patients with low serum thymidinkinase seem to show a better response to 2-CDA. The efficacy of the 2-CDA therapy was influenced neither by the duration of the disease nor by the number and type of other previously administered drugs. There was, however, some evidence of a worse response to 2-CDA in patients with B symptoms and/or pathological changes in peripheral blood and/or in the bone marrow. In comparison to interferon alpha 2-CDA showed a better efficacy with regard to introduction and duration of remission. The main side effect of 2-CDA therapy was a partly severe myelosuppression. Fever, mild infections and a marked tendency to drug induced rushes occured rather often. Two patients developped progredient neurological disorder. One of them showed disarthria, the other disbasia. Since both patients had already similar, but slight neurological signs before 2-CDA treatment there is no clear causal relation between the 2-CDA therapy and these neurological symptoms. In accordance with the literature 2-CDA proves to be a very efficient drug in hairy cell leukemia with tolerable side effects.
