Abstract:
Nowadays, integrated care is widely acknowledged as a promising approach for patient-centric treatments, for which adequate software support is still missing. This thesis addresses the primary integrated care challenges with adaptive case management. It enables modeling treatment-specific case templates and allows patient-centric run-time adaptions where required, with purely meta-model-based integration patterns that enable treatment- and hospital-specific customization, and with seamlessly incorporated case-based collaboration capabilities.

Heutzutage wird die integrierte Versorgung als vielversprechender Ansatz für patientenorientierte Behandlungen anerkannt, für die eine angemessene Softwareunterstützung noch fehlt. Diese Arbeit adressiert die primären Herausforderungen der integrierten Versorgung mit adaptivem Fallmanagement. Es ermöglicht die Modellierung behandlungsspezifischer Fallvorlagen und bei Bedarf patientenzentrierte Laufzeitanpassungen mit rein metamodellbasierten Integrationsmustern, die behandlungs- und krankenhausspezifische Anpassungen ermöglichen und mit nahtlos integrierten, fallbezogenen Kollaborationsmöglichkeiten.
