Management of postoperative and postinterventional bile duct complications

Iatrogenic bile duct complications after laparoscopic cholecystectomies or endoscopic interventions are associated with high rates of morbidity and mortality. Several classification systems have been proposed to categorize these lesions, but non has gained universal acceptance. Early identification and treatment is associated with a significant improvement in prognosis. The management of iatrogenic bile duct complications depends on the severity, the type and the localization of the injury, but also on the time of diagnosis. Management of these injuries should be restricted to specialized centers with high expertise in hepatobiliary surgery, endoscopy and interventional radiology. Besides endoscopic procedures and percutaneous transhepatic interventions, surgical reconstruction is of utmost importance in the therapy of these patients. Most patients need long-term follow-up. An interdisciplinary collaboration between surgeons, endoscopists and interventional radiologists achieves the best results.     «
