Extracellular vesicles (EVs) are crucial mediators of intercellular communication and promising diagnostic targets in liquid biopsies. This work aimed at assessing the applicability and utility of exploiting EVs and their microRNA cargo as clinical biomarkers in vitro and in vivo. Next-Generation Sequencing-based analysis of microRNAs in cohorts of sepsis patients and cardiovascular disease patients demonstrated the diagnostic and prognostic potential of EVs. Additionally, challenges for the development of EV-based biomarkers and approaches to overcome them were identified.
