Im Rahmen der Diskussion über die Einführung von DRGs in die Leistungsabrechnung der deutschen Kliniken befassen sich nur wenige Publikationen mit dem Problem der erheblichen Mehrarbeit der Ärzte im administrativen Bereich, welche v.a. durch Verschlüsselung der Diagnosen, Erfassung der Prozeduren und durch Supervision derselben sowie Schulungen zur Materie zustande kommt. Ziel der dieser Arbeit zugrunde liegenden prospektiven Studie war es, die durch Verschlüsselung und Supervision der Diagnosen und Prozeduren entstehende zeitliche Mehrbelastung in einer Abteilung für Plastische Chirurgie objektiv zu erfassen und beispielhaft darzustellen. Aufgrund der objektiv ermittelten Daten wird in dieser Arbeit die steigende Arbeitsbelastung der Ärzte durch die reinen Dokumentationsaufgaben dargestellt. Auf Basis dieser Erhebungen und von Literatur werden allgemeine Folgen der DRG-Einführung und dieser Mehrarbeit für die Plastische Chirurgie und andere Bereiche der heutigen Hochleistungsmedizin diskutiert.     «

While the discussion about introduction of DRGs into german hospital´s accounting of medical services is still running, there are only few publications about the rising additional work for physicians, which is mainly caused by coding diagnoses, procedures and the supervision of it as by the training for this subject. The task of this study was to objectively collect data about the additional work and time lag for physicians caused by the coding and supervising of diagnoses and procedures exemplary in a dept. for plastic surgery. In this work on the basis of these objectively collected data the additional work for physicians caused exclusively by the pure documentary responsibilities is shown. Upon these science-based uprisings and upon literature the implications of the introduction of DRGs and of the additional work for plastic surgery and other fields of the modern high-level-medicine are shown and discussed.      «
