We examined the influence of positive surgical margins (R1) on several pathological and clinical parameters in prostate cancer with pathological stage T2 or T3 on progression free survival. Patients with a pT2R1-tumor had no higher risk for PSA-progression than patients with pT2R0. Patients with pT2R1 exhibited higher median PSA at diagnosis, and a higher proportion of tumors with poor differentiation than patients with pT2R0. Progression free survival was in the pT2R0 and pT2R1 groups significantly higher than in patients with a pT3-tumor. Based on these results prostate cancer specimen with positive surgical margins and no extracapular extension should probably neither be classified as T2 nor as T3 disease.
