Osteochondral lesions are often caused by chronic knee instability and varus malalignment. In this study we presented the indications, operative techniques and results for the combined operation of high tibial osteotomy and cruciate ligament reconstruction. Between 1996 to 2001, 68 patients were treated by this procedure. 40 persons (group I, average age 38 years) underwent the first cruciate ligament reconstruction, 28 patients (group II, average age 41 years) had a revision cruciate ligament reconstruction. On the one hand, we investigated the clinical and subjective changes secondary to the operation, on the other hand, postoperative results were analyzed radiologically. In conclusion, despite subtle radiological changes, nearly all patients reported an improvement of discomfort. Therefore this operative technique can be considered as a sufficient therapy
