Die chronische lymphatische Leukämie vom B-Zelltyp (B-CLL) ist charakterisiert durch fortschreitende maligne Akkumulation kleiner CD5 positiver B-CLL Zellen im peripheren Blut. Obwohl die peripheren B-CLL Zellen in der G0/frühen G1 Phase des Zellzyklus arretiert sind, werden proliferierende leukämische Zellen beschrieben, vorkommend als Pseudofollikel in so genannten Proliferationszentren in Lymphknoten und Knochenmark. Diese proliferativ aktiven Zellen scheinen von Bedeutung in Bezug auf die Progression der Erkrankung zu sein. Sie können sowohl Ausgangspunkt für Rezidive darstellen als auch den Pool der peripheren CLL Zellen erneuern. In der vorliegenden Arbeit wird anhand eines in vitro Modells (B-CLL Zellen stimuliert mit CpG-Oligonukleotiden zusammen mit Interleukin 2) der proliferative Anteil der B-CLL untersucht. Lactacystin, ein mikrobieller Metabolit, hemmt spezifisch die proteolytische Aktivität des Proteasoms. In dieser Arbeit wird gezeigt, dass Lactacystin in der Lage ist, hochselektiv in die Zellzyklus- und Apoptoseregulation sowohl proliferierender als auch ruhender B-CLL Zellen einzugreifen. Lactacystin inhibiert die Degradierung des cyclin dependent kinase (CDK) Inhibitors p27 bei aktivierten B-CLL Zellen. Die Hochregulation von Zyklin D2 und Zyklin D3 wird inhibiert, wohingegen die Expression von CDK2, CDK4 und Zyklin E unbeeinflusst bleibt. Lactacystin löst bei stimulierten als auch ruhenden B-CLL Zellen Apoptose aus. Dabei reagieren aktivierte B-CLL Zellen signifikant sensibler auf Lactacystin als ruhende B-CLL Zellen. Die Apoptoseinduktion wird begleitet durch den kompletten Verlust des mitochondrialen Membranpotentials. Desweiteren tritt eine Cleavage von Procaspase 3 auf. Z-VAD.fmk, ein Pancaspaseinhibitor, ist in der Lage die Cleavage von Procaspase 3 komplett zu hemmen, die Apoptoseinduktion jedoch nur zum Teil. Ein als proapoptotisches Protein beschriebenes 18kD Cleavageprodukt von bax akkumuliert bei mit Lactacystin behandelten Zellen. Im Gegensatz dazu kommt es zu keiner Änderung der Expression oder Cleavage von bcl-2. Die Cleavage von bax kann weder durch z-VAD.fmk, noch durch Calpeptin, einem Calpaininhibitor, gehemmt werden. Die Inhibition des Proteasoms durch Lactacystin löst hoch effektiv Apoptose bei proliferierenden B-CLL Zellen aus. Dabei werden caspaseabhängige sowie unabhängige Wege benutzt. Die Ergebnisse der Arbeit zeigen, dass Therapeutika mit Aktivität gegen das proliferative Kompartiment der B-CLL eine attraktive Ergänzung in der Therapie dieser meist unheilbaren Erkrankung darstellen können.     «

B cell chronic lymphocytic leukemia (B-CLL) is characterized by malignant accumulation of small CD5 positive lymphocytes in the peripheral blood. Despite the proliferative inactivity of these cells who are arrested in G0/early G1 phase of cell cycle, proliferating leukemic cells have been described in so called proliferation centres in lymph nodes and the bone marrow. These proliferative active cells seem to be of importance for disease progression because they may be responsible for disease recurrence and for refilling the proliferative inactive B-CLL cells in the peripheral blood. In the present study the proliferative active pool in B-CLL is investigated on the basis of an in vitro model for stimulated B-CLL cells (B-CLL cells stimulated with CpG-oligonucleotides together with interleukin 2). Lactacystin, a microbial derived metabolite, inhibits specifically the proteolytic activity of the proteasome. This study demonstrates the highly selective interference of lactacystin with cell cycle and apoptosis regulation of proliferative active and inactive B-CLL cells. Lactacystin inhibits the degradation of cyclin dependent kinase (cdk) inhibitor p27 in proliferative active B-CLL cells. The upregulation of cyclin D2 and D3 is inhibited, whereas expression of cdk2, cdk4 and cyclin E remains unchanged. Lactacystin induces apoptosis in proliferative active and inactive B-CLL cells though activated cell are significantly more susceptible than non activated cells. Apoptosis induction is accompanied by the complete loss of mitochondrial membrane potential as well as cleavage of procaspase 3. Z-VAD.fmk, a pan caspase inhibitor, is able to completely inhibit cleavage of procaspase 3. However, apoptosis induction is prevented only in part. A recently described, proapoptotic 18kDa cleavage product of bax accumulates upon treatment with Lactacystin. In contrast no change in expression or cleavage of bcl-2 is detectable. Cleavage of bax can not be inhibited by z-VAD.fmk or calpeptin, a calpain inhibitor. The inhibition of the proteasome by Lactacystin induces highly effective apoptosis in proliferating B-CLL cells via caspase dependent and independent pathways. The results of this study indicate that therapeutics active against the proliferative compartment of B-CLL represent an attractive completion in the treatment of this mainly incurable disease.     «
