In this prospective study, the radiological and clinical outcome of 32 patients after cervical spine surgery was investigated concerning global and cervical sagittal balance. Cervical sagittal balance only showed radiological correlations with global alignment. Global sagittal balance was related to clinical parameters – but not postoperatively. Overall spinal and pelvic alignment should be used to evaluate sagittal balance. Common compensatory mechanisms of sagittal imbalance are seen in mismatch values and retroversion of the pelvis.
