The aim of this retrospective study was the clincal examination of the mechanical and functional stability between different techniques of anterior cruciate ligament reconstructions two years post-surgery. Therefore three different surgical techniques were examined:
• Patellar tendon Autograft in Singlebundle technique (PS-SB)
• Semitendinosus-/Gracilis Autograft in Singlebundle technique (STG-SB)
• Semitendinosus-/Gracilis Autograft in Doublebundle technique (STG-DB)
Beyond that, the patients were divided into Coper (C) and Non Coper (NC), thus in there ability to compensate an instability after rupture of the anterior cruciate ligament. For the division in Coper and Non Coper the Hop Test, the Activities of Daily Living Score, the Global Rating of Knee function and the occurrence of instability feelings were the basis.
A total of 66 patients were enclosed in the study, 22 in the PS-SB group, 19 in the STG-SB group and 25 in the STG-DB group. The group distribution was homogeneous according to the statistical analysis. Nevertheless, the demanded power of the groups of 25 patients could only be accomplished in the STG-DB group.
In summary it can be said, that ,when the comparing the three surgical techniques, no significant differences were to be identified. With the division in Coper and Non Coper a significant superiority could be shown in the subjective tests (IKDC subjective, Lysholm/Gillquist Score), as in the OAK Score Category C (Stability), an objective examination procedure. Also in the total OAK Score the Coper performed better.
It remains to be examined in further studies, whether a preoperative division into Coper and Non Coper may lead to a stricter medical indication of surgery. In addition studies should be implemented to find out whether an anatomical position of the drilling channels could lead to better functional and mechanical results.
