By means of stereotactic fractionated radiotherapy, recurrences of high-grade gliomas can be treated by re-irradiation. The present study confirmed the efficacy of this approach, leading to mean survival of 37,0 months after primary diagnosis (n = 48). Analyses at the time of glioma recurrence revealed the following significant positive influences: young age, surgical tumor resection, simultaneous radiochemotherapy, and lower histological grade according to WHO classification. Although some some structures at risk received high total radiation doses, no relevant side effects were observed.
