An early differentiation between various aetiologies of kidney injury is therapeutically relevant. In a prospective study, fractional excretions of natrium (FES) and urea (FEU) of 36 patients were analyzed for differentiating. To validate results, the global end-diastolic volume index (GEDVI), calculated by transpulmonary thermodilution, was used. Decreases in GEDVI and decreases in FES/ FEU are not interchangeable. Alterations in FEU were significantly correlated with changes in serum creatinine. Every akute kidney injury showed decreased FEU, but only 45% showed concomitiant decreased GEDVI.
