In 2009, the 7th edition of the TNM classification for colorectal cancer was published, with the aim of improving prediction of prognosis. The complexity of the classification increased simultaneously.
Between 1990 and 2006, 2.229 patients underwent colorectal cancer surgery at the Klinikum rechts der Isar, TU München. This data sets allowed comparison of the prognostic value of the former 6th, of the new 7th and of another prognosic model based on clinical and histopathological factors.
Only 7% of all patients had a stage shift in the new TNM classification. Despite small effects in distinct subgroups, there was no relevant difference between the prediction of prognosis regarding the 6th and the 7th edition as well as the independent prognostic model.
Despite increased complexity, the new TNM classification did not reach the aim of providing a better prognostic model.
