17 patients had been administered 200 mg ofloxacin intravenously three hours before starting surgery. During the operation ligamentum flavum, disc tissue, disc sequestrum(not in all patients), liquor as well as blood at operation start and operation end have been taken from the patient to determine the concentration of ofloxacin in the tissue samples collected and in the serum. The ofloxacin concen-trations measured were partly below the required value of 1µg/ml, in the bradytrophic tissues (disc,sequestrum) and in liquor significantly lower than in ligamentum flavum which is significantly better supplied with blood. In comparison to the ofloxacin concentrations in the tissue samples consistently higher antibiotic concentrations were found in the extravas-cular space. Although the antibiotic concentrations measured in relation to the MIC of staphylococci are comparable to those of cephalosporins of the 2nd generation, ofloxacin cannot become accepted as medicine for perioperative infection prophylaxis. This is due to the disadvantageous range of side effects of ofloxacin as compared to cephalosporins and the rapid development of resistance of staphylococci to the antibiotic.
