From January 2002 to December 2006, 64 patients underwent carotid endarterectomies based on an anatomy-adapted patch closure as modified procedure of the carotid surgery at the clinical centre of München-Schwabing. There were 36 asymptomatic and 28 symptomatic carotid stenoses. 17 vein and 47 dacron patches were used. After the arteriotomy on the CCA a primary closure was performed.
The perioperative morbidity corresponded to the results published in prospective randomized studies. As a long-term result, averaging 36 months, 44 patients had duplex scanning.
The CEA with anatomy-adapted patch closure proved to be reliable for the angiomorphological and neurological outcomes. The restenosis rates remained within the expected value of comparable collectives. An adequate selection of patients based on anatomical properties of the carotid bifurcation has proved to be an alternative to other surgical methods.
