239 patients suspected of choledocholithiasis have initialy been examined by EUS.
In case of stone finding (32,9%) a therapeutic ERCP followed. In the absence of stones, no ERCP was performed, the patients were followed. The EUS showed a high diagnostic accuracy concerning a choledocholithiasis (sensitivity 89,7%; 100% specificity). There were no EUS associated complications.
129 ERCPs and therefore 6,3 complications could be avoided.
Compared with the conventional diagnostic ERCP the EUS guided diagnostic resulted same benefits with lower costs.
The EUS based algorithm is recommended because of clinical and economic reasons.
