Bei der operativen Versorgung von Femurschaftfrakturen gilt die intramedulläre Stabilisierung heute als Verfahren der Wahl. Die Arbeit beinhaltet eine vergleichende Analyse von Osteosynthesen mit zwei verschiedenen Nageltypen (34 SIRUS-Nägel, Firma Zimmer, 34 UFN, Firma Synthes). Der richtige Eintrittspunkt für den Nagel in das Femur ist von hoher Relevanz für das operative Ergebnis und variiert in Abhängigkeit von Nageldesign und- produkt. Beim SIRUS-Nagel ist der laterale Insertionspunkt am Trochanter major leicht zu identifizieren und kann über eine begrenzte Weichteilfreilegung erreicht werden. Zugangsassoziierte Komplikationen waren nur in der UFN-Gruppe zu beklagen (2 iatrogene Schenkelhalsfrakturen, 1 posttraumatische Femurkopfnekrose). Die klinischen, radiologischen und funktionellen Langzeitergebnisse zeigten sich in beiden Gruppen identisch.      «

The intramedullary stabilization is seen as the preferential method in care of femoral fractures. This work contains a comparative analysis of osteosynthesis with two different types of nail (34 SIRUS-nails, 34 UFN, Synthes enterprise). The right spot of access for nail into the femur is highly relevant for the result of surgery and varies due to design and type of nail. Using the SIRUS-nail, the insertion point at the trochanter major is easy to identify and can be easily accessed with a minimum of soft part exposure. Complications associated with access only appeared among the UFN-group (2 iatrogenic femur neck fractures, 1 posttraumatic necrosis of the femoral head). The clinical, radiologic and functional long term results were identical among both groups.     «
