Die PVNS ist eine seltene Erkrankung, die meist große Gelenke betrifft. Bis heute gibt es keinen einheitlichen Therapiestandard. Im Zeitraum 1997-2008 wurden von der Abteilung für Orthopädie und Sportorthopädie des Klinikums Rechts der Isar 45 Patienten mit PVNS behandelt. Ziel dieser Arbeit war es, in Abhängigkeit von Befallsform und Therapie, Rezidivhäufigkeit und postoperatives Outcome zu ermitteln, sowie einen Therapiealgorithmus zu erstellen. Beim Erstbefall der diffusen PVNS des Kniegelenkes sollte eine offene Synovektomie von ventral, ggf. mit einer dorsalen kombiniert werden. Bei Rezidiven kann eine adjuvante RTX in Betracht gezogen werden.     «

Pigmented Villonodular Synovitis is a rare disease that mainly affects great joints. Until today, there is no standard concept for operative treatment, arthroscopic and open synovectomy are discussed. This study presents the results of 45 patients that were treated for PVNS in the Department of Orthopedics and Sports Medicine of “Klinikum Rechts der Isar” between 1997 and 2008. The aim of this study was to show the postoperative Outcome and recurrence rate regarding the operative treatment and the localization; and to create a treatment algorithm Combined Synovectomy and radiation leads to a good clinical outcome and has the lowest recurrence rate. Open complete synovectomy of the knee or combined arthroscopic and open synovectomy with adjuvant radiation therapy might be the method of choice for treatment of severe cases of (recurrent) diffuse PVNS.     «
