It is still debated whether the synovial biopsy must be undertaken under direct arthroscopic vision or can be performed without visual control. Counting twice as many non-representative histological samples is a limitation of the blind biopsy. Compared with the gold standard (intraoperative samples upon prosthesis removal), the biopsy shows independent of the method a sensitivity of 50%, specificity of 64.3%, NPV of 81.8% and PPV of 28.6%. In summary, an adaptation of the diagnostic algorithm is needed. At present, sampling should continue under arthroscopic vision.     «
