The data of 3031 patients, undergoing open heart surgery in the German Heart Center Munich, were analysed. Those patients who did not spend autologous blood had an incidence of 64% for homologous transfusion. By the means of preoperative donation of 2,5 units of autologous blood a reduction to 16% could be achieved. An average of 1,8 units of homologous blood and 62,49 DM of transfusion costs per autologous blood donor could be saved. Comparing the need of transfusion among different groups of patients, the decision tree analysis is a convenient means to get specific and individual information. It also helps improving clinical decision in transfusion practice, also on a financial level. Autologous blood donation is a save and eligible alternative to homolous blood donation.
