We assessed the functional outcome of 100 patients with oropharyngeal squamous cell carcinoma. 50 patients received a transmandibular resection with a radial forearm flap reconstruction and an additional chemoradiotherapy (TMR+CRT) compared to 50 patients receiving primary chemoradiotherapy (pCRT). In the water swallowing test, the primary endpoint of the study, the patients treated with pCRT showed a better performance. However, after modifying the surgical reconstruction technique the swallowing performance was more comparable to the pCRT group. Furthermore we assessed nasal penetration, xerostomia, speech and quality of life.
