Patienten-Prothesen-Mismatch ist ein Missverhältnis zwischen optimaler und real implantierter Größe einer Prothese nach Aortenklappenersatz. Der Einfluss von Patienten-Prothesen-Mismatch auf die Überlebenszeit wird kontrovers diskutiert. In der vorliegenden Studie wurden 645 Patienten, die sich einer Aortenklappenoperation unterzogen hatten, bis zu fünf Jahre nachuntersucht. In bisherigen Untersuchungen wurde Patienten-Prothesen-Mismatch nur über Mittelwerttabellen geschätzt. In dieser Arbeit liegt durch direkte Messung der Klappenöffnungsfläche mithilfe der Echokardiografie in allen Patienten eine sehr genaue Methode vor. Es konnte mit den gesammelten Daten und den angewendeten Methoden kein signifikanter Einfluss von Patienten-Prothesen-Mismatch auf die Überlebenszeit gesichert werden. Die bekannten Kofaktoren einer erhöhten Mortalität kardialer Patienten (z.B. Diabetes mellitus, koronare Herzerkrankung, chronisch obstruktive Lungenerkrankung) konnten auch im untersuchten Patientengut bestätigt werden.     «

The prosthesis-patient mismatch (PPM) represents the disproportion between the optimal and the real size of the implanted prosthesis after aortic valve replacement (AVR). The influence of prosthesis-patient mismatch on the survival time is discussed controversially. In the present study 645 patients, who recently underwent an aortic valve operation, have been followed-up up to 5 years. In previous publishing’s the prosthesis-patient mismatch were estimated only with reference tables. In the following study exists with direct measurement of the effective orifice area through the echocardiography by all patients a very exact method. With the collected information and methods used it did not reach that a statistical significant influence of prosthesis-patient mismatch on the survival time exists. The known covariates of high mortality of cardiac patients (e. g. diabetes mellitus, coronary heart disease, chronic obstructive pulmonary disease) could also within the study population confirmed.     «
