The exact risk classification in primary breast cancer patients with 1-3 involved axillary lymph nodes is still under discussion. In this MD thesis, the significance of established prognostic factors (number of involved lymph nodes, grade, tumor size, steroid hormone receptor and menopausal status) and that of tumor biological factors (uPA, PAI-1) was examined - considering adjuvant systemic therapy - regarding course of disease (n=182). Number of involved lymph nodes and grade were the only significant prognostic factors for disease-free survival; for overall survival, grade, invasion marker uPA, number of involved lymph nodes and combination of uPA/PAI 1 were significant. Adjuvant anthracycline-containing chemotherapy seemed to be particularly beneficial. These results provided a basis for the German EC-DOC- trial, which aimed at optimization of adjuvant chemotherapy for this patient group.
