This study aimed to define dermatohistopathological criteria for the diagnosis of mastocytosis in the skin (MIS). 41 MIS cases and 36 controls were used for immunohistochemical stainings and evaluated microscopically. The following criteria were found to be most important: Mast cell (MC) density in the upper 300 µm of the skin (cut-off 140 MC/mm
2 , specificity = 97,2%), intermediate to strong basal hyperpigmentation and interstitial occurrence of MCs or MC clusters. T cells were commonly found, but are unspecific.
