Background: Capsular contracture is the most common complication after breast implant surgery leading to a revision surgery. This pilot-study evaluates the influence of different surface topographies on the reduction of capsular contracture in a Minipig model.
Materials and Methods: 6 female Goettingen Minipigs were implanted with altogether 12 silicone implants of each type, smooth (S), textured small-pores (SP) and textured large-pores (LP), creating separate subglandular pockets for each implant. The implants and their fibrous capsules were compared at 10- and 20-week time points by clinical examination, histology/immunohistochemistry, scanning electron microscopy, microbiology and biomechanics.
Results: The most important finding of this study was the correlation between the surface topography of the breast implants and the tissue response. The textured small-pores implants showed significantly softer and thinner fibrous capsules than textured large-pores surfaces by biomechanics and histology. These findings correlated well with the degree of the Baker score. The immunohistochemistry showed a higher expression of integrin-ß1, which is a marker for cell-cell-adhesions, in SP-capsules compared to LP-capsules at the implant side. However, these effects were not significantly different in comparison between SP- and S-implant surfaces at the 20-week time point. On the other hand, capsules of textured implants showed a different histomorphological structure than of smooth implants. Following, it could not be excluded that the textured surfaces could lead to a reduction of capsular contracture during a longer follow-up and larger numbers of cases. This study established for the first time successfully the Goettingen Minipig model for the evaluation of capsular contracture in breast implants.
