In wheat-dependent exercise-induced anaphylaxis (WDEIA) diagnosis is often difficult. In 16 patients with WDEIA a new challenge protocol with gluten plus cofactors was established. In all patients, the diagnosis could be detected. Reactions were associated with high plasma gliadin levels. Cofactors led to increased gastrointestinal permeability. The reaction threshold was not correlated with immunological parameters. An oral challenge test with gluten plus cofactors is necessary to confirm a WDEIA.
