Since the importance of the medial patellofemoral ligament (MPFL) as the main passive stabilizer is known, its reconstruction has been established as an operative therapy for patellofemoral instability.
This prospective study aims to investigate subjective, clinical and radiological findings before and after MPFL reconstruction with a gracilis autograft in double bundle technique. Furthermore, risk factors are considered and the outcome is differentiated between initial and revision surgery as well as between isolated and combined MPFL reconstruction.
The minimally invasive MPFL reconstruction shows overall excellent results without any redislocation. Moreover, a previous non-anatomic surgical procedure leads to a worse preoperative starting condition. The significantly better outcome of initial surgery compared to revision surgery induces that the MPFL reconstruction should be the first choice in PFI-surgery and should be performed during combined procedures in multiple factorial instability.
