In a cohort of 134 resected intraductal papillary mucinous neoplasms (IPMNs), the global prevalence of preoperative diabetes (DM) proved to be high. DM was significantly associated with main-duct involvement and high-grade dysplasia/invasive carcinoma. The risk of malignancy was even higher in patients with new-onset/worsening DM. Moreover, patients with weight loss at diagnosis had more frequently DM, but only in presence of malignancy. In conclusion, DM in IPMN reflects a more aggressive disease and may be helpful for early diagnosis and prevention of invasive cancer.

In einer Kohorte von 134 resezierten intraduktal papillär muzinösen Neoplasien (IPMN) erwies sich die Prävalenz von präoperativem Diabetes mellitus (DM) als erhöht. Dabei war DM war signifikant mit einer Hauptgangsbeteiligung und hochgradiger Dysplasie/invasivem Karzinom assoziiert. Das Malignitätsrisiko war bei Patienten mit neu aufgetretenem oder sich verschlechterndem DM sogar noch höher. Außerdem hatten Patienten mit einem Gewichtsverlust bei der Diagnose häufiger DM, dies jedoch nur bei Vorliegen von Malignität. Zusammenfassend lässt sich sagen, dass DM bei IPMN eine aggressivere Erkrankung widerspiegelt und für die Frühdiagnose und Prävention eines invasiven Karzinoms hilfreich sein kann.
