Abstract:
Mit der Bergung von Thromben durch gezielte Thrombektomien als Therapie von akuten Schlaganfällen ergab sich die Möglichkeit der histopathologischen Analyse von menschlichen Thromben, unter anderem, zur genaueren Zuordnung der Ätiologie des Infarkts. Mechanisch geborgene Thromben von kardioembolischen und nicht-kardioembolischen Schlaganfallpatienten unterscheiden sich signifikant in ihrer histopathologischen Zusammensetzung. Kryptogene und kardioembolische Thromben hingegen zeigen deutliche Ähnlichkeiten sowohl in ihrer histopathologischen Zusammensetzung als auch auf interventioneller und klinischer Ebene. Unsere Ergebnisse legen nahe, dass der Großteil kryptogener Schlaganfälle kardioembolischer Genese sind.

With targeted thrombus-extracting devices as a acute stroke therapy it became possible to analyse the histopathology of human thrombi also to identify specific causes of a stroke. Mechanical extracted thrombi of cardioembolic and non-cardioembolic stroke patients show different histopathological components. Thrombi from cryptogenic stroke patients show the same basic pattern as cardioembolic thrombi in terms of thrombus composition, interventional and clinical level. Our results suggest that the majority of cryptogenic strokes have a cardioembolic origin.
