Abstract:
Die Studie verglich das geschätzte Herzzeitvolumen (HZV) aus der Pulskonturanalyse (PKA; PiCCOplus- bzw. FloTrac-System) mit exakten HZV-Werten der transkardiopulmonalen Thermodilution (TPTD). Nach einer einmaligen initialen Kalibrierung der PiCCO-PKA wurden PiCCO- und FloTrac-PKA-HZV von 28 Patienten mit 4-stündlichen TPTD-HZV verglichen. Die PiCCO-PKA ergab eine bessere Übereinstimmung mit dem TPTD-HZV als die FloTrac-PKA. Die Genauigkeit des PKA-HZV beider Systeme hing nicht vom Zeitabstand zur Kalibration ab, als vielmehr von großen Schwankungen in der Hämodynamik. Eine Kalibration der FloTrac-PKA ergäbe eine genauere Überwachung.

We examined the stability of two pulse contour analysis devices (PCA; PiCCOplus- and FloTrac-system) measuring cardiac output (CO) compared to transcardiopulmonal thermodilution (TPTD) in 28 critically ill patients. After an initial calibration of the PiCCO-PCA we compared the two devices with TPTD-COs every 4 hours without recalibration. The PiCCO-PCA showed a better match than the FloTrac-PCA. A fixed recalibration frequency can't be specified, but large changes in patient's hemodynamic situation should entail recalibration. If the FloTrac-PCA received an initial calibration, we would expect a better performance.
