In a cohort of 134 resected intraductal papillary mucinous neoplasms (IPMNs), the global prevalence of preoperative diabetes (DM) proved to be high. DM was significantly associated with main-duct involvement and high-grade dysplasia/invasive carcinoma. The risk of malignancy was even higher in patients with new-onset/worsening DM. Moreover, patients with weight loss at diagnosis had more frequently DM, but only in presence of malignancy. In conclusion, DM in IPMN reflects a more aggressive disease and may be helpful for early diagnosis and prevention of invasive cancer.
