Kurzfassung:
Die Therapie von Kopf-Hals-Karzinomen hat den Balanceakt zwischen ausreichender onkologischer Radikalität und Erhalt bzw. Wiederherstellung der Funktionalität zu meistern. Vor allem positive wie unklare Resektionsränder und die extrakapsuläre Tumorausbreitung sind für Verlauf, Therapie und Überlebenschancen prognostisch entscheidend und beeinflussen die Therapieintensität.
Die vorliegende Studie hat retrospektiv für 534 Patienten ECE- und R-Status untersucht und Behandlungskonzepte analysiert. Dabei wurde häufiger postoperativ mit Radio(chemo-)therapie behandelt, als es Personen mit positivem ECE-Status gab. Auch bei unklarem Resektionsrand wurde oftmals eine intensivere Therapie gewählt.

Therapy of head and neck malignancies have to manage the balancing act between radicality and the preservation of functional structures. In particular, unclear resection margins and the extracapsular tumor spread are prognostic factors for process, therapy and chances of survival and influence treatment intensity.
The present study retrospectively examined ECE and R status for 534 patients and analyzed their treatment concepts. Postoperative treatment with radio (chemo) therapy was more common than there were people with a positive ECE status. Overtreatment was also frequently performed with unclear resection margins.
